Deathkings of the Dark Citadel est la seule extension officielle du jeu original HeXen. Elle comprend 20 nouveaux niveaux en solo conçus par l’équipe d’origine ainsi que diverses options multijoueur.
Cet add-on marque la première véritable extension créée par Raven Software. En plus d’ajouter trois nouveaux hubs pour HeXen, il a permis pour la première fois à huit joueurs de se connecter simultanément en réseau local.
